A US-Israeli airstrike killed Ali Khamenei, Iran's Supreme Leader, on February 28, 2026 — and wounded his son Mojtaba. The Assembly of Experts named Mojtaba the new Supreme Leader on March 9. He has not made a single public appearance — no video, no audio, no images — since that airstrike. That's 132 days. He skipped his father's state funeral in Qom last week, while his three brothers attended. He has issued three written statements — the first, on March 12, was read aloud by a state TV presenter over a still photo — and none has ever included his voice or image. Nuclear negotiations with the US are scheduled in Islamabad tomorrow, July 11.

1. Iran Says He's Managing From the Back (Mozaher Hosseini, Masoud Pezeshkian)

The official Iranian position is that his injuries are minor and he is actively governing — just not publicly.

Iran's injury story keeps getting longer — not more credible. Iran's Health Ministry said on March 1 that Mojtaba had superficial injuries, was stitched up, and discharged from the hospital the same day. Since then, officials have released four successive rounds of medical detail, each acknowledging a bit more injury while insisting everything remains minor. Mozaher Hosseini, chief of protocol for the Supreme Leader's office, says Mojtaba is in "complete health" — just a shrapnel fragment behind his ear, a foot injury, a lower-back issue.

Iran's president claims he met Mojtaba in person. President Pezeshkian — a cardiac surgeon — announced in May 2026 that he'd held an extended in-person meeting with Mojtaba. General Ali Abdollah, the armed forces joint operational command chief, has also claimed a personal meeting. These are the only two known official claims that anyone has seen him face to face since March.

The written statements project defiance without ever producing him. All three of Mojtaba's statements — from March, April, and May — have carried strong language: US bases will have no place in the region, Iran won't forfeit its rights, Iran will keep striking any US installation that hosts attacks. The regime is clearly communicating. Whether Mojtaba is actually the one doing it — nobody can say.

2. But the Injury Reports Keep Getting Worse (Reuters, New York Times, Pete Hegseth)

Three independent reporting streams — Reuters, the NYT, and US officials — all describe severe incapacitation.

He lost a leg and suffered severe facial burns. Three people close to Mojtaba's inner circle told Reuters (published July 5) that his face was disfigured and that he suffered a significant injury to one or both legs. A US intelligence source told Reuters separately that he was believed to have lost a leg. Trump said publicly in mid-March that Mojtaba was badly disfigured, had likely lost a leg, and — he added — that he didn't even know if Mojtaba was still alive. Defense Secretary Hegseth said on March 13 he was "wounded and likely disfigured."

The New York Times got more specific. Four senior Iranian officials told the NYT in late April that Mojtaba's leg was operated on three times, that he's awaiting a prosthetic, that he underwent hand surgery with slow recovery, that he has severe facial burns requiring future plastic surgery, and that his lips were burned badly enough to make speech difficult. Iranian opposition figure Mohsen Sazegara — a founding IRGC member turned regime critic — said his sources confirmed abdominal and leg surgery plus possible facial injuries.

He's mentally sharp, according to these same sources. Multiple sources, including those in the NYT report, say Mojtaba is cognitively engaged and participates in meetings via audio conference. The gap isn't mental capacity — it's physical appearance.

3. And Nobody Can Sign a Nuclear Deal If He Can't Be Seen (Negar Mortazavi, FDD)

Iran's constitution requires the Supreme Leader to personally authorize any treaty-level agreement — and that authority can't be delegated.

Iran's constitution has a specific problem. Under Article 110, the Supreme Leader has exclusive authority over nuclear negotiations and treaty commitments — authority that cannot be handed off to the president or anyone else. If Mojtaba can't appear publicly or speak, how any final nuclear agreement gets signed remains a genuine open question. Nuclear talks are scheduled in Islamabad tomorrow.

In the meantime, five men appear to be running Iran. The Foundation for Defense of Democracies identified the de facto governing coalition in April: IRGC commander Ahmad Vahidi, Supreme National Security Council Secretary Mohammad Bagher Zolghadr, economic coordinator Mohsen Rezaei, Parliament Speaker Mohammad Bagher Ghalibaf, and President Pezeshkian. None of them can constitutionally ratify a deal.

The funeral absence made this impossible to ignore. Political analyst Negar Mortazavi said after the funeral: "They can't hide him forever." The IRGC pressured the Assembly of Experts into naming Mojtaba Supreme Leader — partly because hardliners needed continuity fast — while Israeli airstrikes were hitting the Assembly's Qom building. What they got was a leader who, as of July 10, still cannot be shown to the world.

Where This Lands

Iran's government insists Mojtaba Khamenei is governing — and that his injuries are manageable and private. Three separate reporting streams from Reuters, the New York Times, and US officials paint a picture of someone severely injured, awaiting a prosthetic leg, and unable to speak clearly. What both camps agree on: he has not appeared publicly in 132 days, and tomorrow Iran is supposed to show up to nuclear talks that require a Supreme Leader to sign off on any deal. The governance analysts say the real story isn't his health — it's that Iran may be functionally leaderless on the question that matters most.
