Pennsylvania's measles outbreak has grown to 731 cases across 38 counties, about 140 hospitalizations, and four measles-associated deaths. On Wednesday evening, state Health Secretary Debra Bogen sent a letter to CDC Director Dr. Erica Schwartz formally requesting a CDC Epi-Aid, the agency's emergency field-epidemiology assistance. The letter came with a condition: Pennsylvania will pull the request if the CDC won't "transparently and accurately" acknowledge the state's four deaths. The CDC's own national dashboard currently counts zero measles deaths for 2026, after the agency switched its counting method.

The deaths are real, and recognizing them is the price of trust.

The CDC's silence is hurting the response, Bogen says. Her letter states plainly: "The CDC's choice not to publicly recognize the deaths undermines our response."

She just wants the CDC to use Pennsylvania's own rule. The state calls a death "measles-associated" whenever there's lab or epidemiological evidence of measles, even if a coroner doesn't list measles as the immediate cause on the certificate. That standard predates this outbreak and produced all four determinations.

Vaccine misinformation made this worse, Gov. Shapiro says. He accuses HHS Secretary Robert F. Kennedy Jr. of spreading it and driving down vaccination rates in the state.

2. HHS and CDC Leadership (Spokesperson Grace Davis Jamison, CDC Director Dr. Erica Schwartz)

Pennsylvania attached a political condition to a public health request, and that's the real problem.

The demand itself is the offense, HHS says. Spokesperson Grace Davis Jamison said, "CDC will not submit to political blackmail that conditions critical public health assistance." She added that the agency "will not allow Governor Shapiro to politicize this process."

Pennsylvania skipped the normal process, the CDC says. The agency argues the state didn't submit its Epi-Aid request through the CDC's standard channels.

The agency's new counting method is stricter by design. The CDC shifted from a broader disease-surveillance system to the National Center for Health Statistics database, which only counts a death as measles-caused if a death certificate lists measles as the underlying cause.

3. Independent and Former Public Health Officials (Demetre Daskalakis, former director, CDC's National Center for Immunization and Respiratory Diseases)

The counting method is the wrong tool for an active outbreak, no matter who's counting.

The CDC picked the wrong tool for the job, Daskalakis says. The National Center for Health Statistics tracks long-term mortality trends, not real-time deaths during an active outbreak. He called the CDC's approach "highly atypical."

He'd raise the same objection no matter who was asking. Daskalakis isn't taking Pennsylvania's side in the negotiation. He's objecting to how the CDC is re-litigating its own established death-counting practice. He calls it "highly unusual" and warns the effect is to make a growing outbreak's toll look smaller than it is.

Pennsylvania has the deaths, the case count, and a growing outbreak; the CDC has a new counting method and a national dashboard that shows zero. Bogen says recognition is the price of federal help. HHS says the condition itself is the problem. Independent former CDC officials say the dispute over method is legitimate on its own, separate from the politics around it. The Epi-Aid request now sits with a deadline attached: Pennsylvania has said it will withdraw it if the CDC doesn't change how it talks about the four deaths.
